In the mouth, plaque continuously forms due to saliva production. Within a few hours after brushing, a fresh layer of plaque can be observed on all teeth and mucous membranes. Plaque naturally attracts bacteria, causing it to thicken over time. The thickening plaque promotes the growth and proliferation of bacteria that can cause inflammation.

Bacterial buildup leads to gingivitis. The most common method to disrupt and remove dental plaque (biofilm) is mechanical cleaning, which involves thorough brushing of all tooth surfaces. If brushing is not sufficiently effective and plaque remains on the teeth, minerals such as calcium, magnesium, and other substances begin to precipitate on the plaque after a few days, causing it to harden into tartar.

Tartar is hard and cannot be removed by brushing. Its surface always contains a fresh layer of plaque and bacteria, which inevitably causes gum inflammation. The rough surface of tartar allows new layers of plaque to adhere more easily, causing it to thicken continuously.

The speed at which plaque transforms into tartar varies among individuals, but it can occur within a few days. Tartar can accumulate above the gum line, but subgingival tartar is also common.

 

Which tooth surfaces are most prone to tartar buildup and why?

 

Tartar is most likely to form on the lingual surfaces of the lower teeth. This is because the salivary glands under the tongue discharge saliva onto this area. Additionally, the narrow shape of the lower front teeth and the difficulty in cleaning the spaces between the teeth contribute to tartar formation.

For similar reasons, the outer surfaces of the upper molars are also prone to tartar buildup.

  • The most important factor is inadequate oral hygiene, specifically the presence of plaque that is not removed from the teeth
  • Irregular, crooked teeth also make cleaning more difficult, which facilitates tartar formation
  • Inadequate nutrition, such as a diet low in fiber and the consumption of sticky, non-chewy foods, can also be a contributing factor. (In such cases, the self-cleaning ability of the tooth surfaces is less effective.)
  • Smoking is also a significant contributing factor.

The most effective method for stopping gingivitis is tartar removal, also known as "tartar cleaning" or scaling, which is primarily performed using ultrasonic scaling devices. This procedure removes not only hard deposits but also fresh plaque. The treatment is carried out by a specialized dental hygienist and is always preceded by a dental consultation with a dentist.

During a periodontal consultation, it may become apparent that tartar removal alone is insufficient due to the existing changes, and other interventions may be necessary for the treatment of periodontitis.

The essence of dental hygiene treatment is to partially use ultrasonic scalers and thin manual tartar removal tools to disrupt and remove plaque and tartar from the surfaces of the teeth, both above and below the gumline. The tooth surfaces are then polished smooth with various polishing devices. If necessary, during the treatments, stubborn stains can be removed from healthy surfaces using specialized sandblasting machines (air-flow polishing with Kavo PROPHYflex).

The procedures generally cause no pain; however, for patients with heightened sensitivity, numbing gel or even local anesthetic injections can be used. The key to effective tartar removal is consistency. Depending on home oral care habits, a thorough professional cleaning is recommended every 3-6 months, accompanied by a periodontal screening or check-up.

After depuration, there are a few characteristics worth noting. Often, the lingual side of the lower teeth feels different than before due to the removal of tartar. This is because the surfaces covered by tartar diminish the sensation of separation between the lower teeth before tartar removal. 

We may feel that the teeth are joined together, and the area feels smoother to the touch. After depuration, in the case of gingivitis, minimal bleeding is a normal occurrence.

After ultrasonic tartar removal, the tooth surfaces regain their original, healthy condition, which may feel unusual at first. Minimal sensitivity is also a normal symptom after depuration. The results can be maintained with proper brushing, which is why our colleagues instruct patients on the correct techniques.

Is it possible to remove tartar at home?

 

We do not recommend attempting to remove tartar at home, as it can cause far more harm than benefit. When deposits have hardened and are no longer just plaque, a toothbrush or interdental brush is insufficient for their removal. Using tools that are harder than a toothbrush poses a risk of injuring the gums or causing oral infections, and unskilled attempts can lead to tissue damage. For these reasons, tartar removal should only be performed in a dental clinic equipped for this purpose.

Is it possible to remove tartar at home?

Tartar removal outside of a dental clinic is not recommended, as it can cause injuries to the gums and teeth in a home environment. Additionally, we cannot ensure proper hygiene conditions at home.

What types of tartar removal devices are used?

Primarily, ultrasonic tartar removal devices are used, complemented by manual tools. Additional methods include air-flow salt polishing (tartar removal powder) and the use of polishing pastes and rubbers.

Is there a mouthwash for tartar removal?

Tartar removal mouthwash does not exist. Mouthwashes are used as antiseptic and anti-inflammatory aids. The bactericidal effect of mouthwash is effective with regular use, but daily use should be avoided.

Is there a toothpaste against tartar?

Such toothpaste is not known. There are commercially available toothpastes recommended for patients with periodontal disease.

Is black tartar on my tooth normal?

Tartar can vary in color, appearing white, yellow, brown, or even black. It is common for smokers to have dark discoloration in their tartar buildup.

Is tartar removal possible only with an ultrasonic cleaner?

In addition to the ultrasonic scaler, manual tools or special air-polishing devices are suitable for breaking up deposits. The latter provides a more pleasant user experience for our patients.

Is it advisable to be gentle with the gums in case of gum inflammation or gum bleeding?

You shouldn't be gentle with the gums; rather, increased mechanical cleaning is necessary, as this will help the gum bleeding subside quickly and restore the gums to a healthy state.
